0:10 Maybe there are other barriers you experience that make it hard to do daily activities.

0:15 If so, you should know tht Colorado’s Medicaid program has services called Long Term Services and Supports or LTSS, as we call it, for short.

0:25 These services exist to help you if you have a chronic illness and/or disability, whether that disability is physical,

0:32 psychological or cognitive, as long as it prevents you from doing things that most other people can do easily.

0:39 LTSS refers to a variety of services that are comparable to services a person might receive in a nursing home, or at a hospital.

0:47 The Colorado’s Medicaid enables us to receive these services in our own homes, or our communities.

0:55 To help, they meet both of our medical and non-medical needs.

0:59 If you apply for these services, you might even hear terms like nursing home level of care, but please don’t worry.

1:05 If you receive LTSS, that does not mean that you have to go to a nursing home, not even for a minute.

1:11 Colorado’s Medicaid agency and disability advocates are proud of our LTSS system

1:17 because we want to make sure people stay in their homes and communities as long as they choose to.

1:22 So with that clarification out of the way, let’s focus on the information that I’m about to share with you about these amazing services.

1:29 So this video will discuss who’s eligible for LTSS. Then we’ll tell you a little bit more about some of the tasks you can get assistance with through the Medicaid Long-Term Services and Support System.

1:41 And finally, we’ll share the steps you take to apply to get Long Term Services and Supports in Colorado.

1:48 So let’s start with who can qualify to receive LTSS. To qualify, you must have a chronic illness or disability that prevents you from being able to care for yourself for long periods of time.

1:59 Your disability may be visible like mine is, but it may also be one that is not immediately visible like a cognitive, neurological or psychiatric disability.

2:08 You can receive these services even if you’re experiencing homelessness.

2:13 If you’re a college student or have to be in another state for a while for your job or any other reason, you can still stay on the program as long as your state of residence remains Colorado.

2:23 There’s also no age requirement to receive these services. They’re available to all people who are eligible, regardless of age.

2:31 There are programs that are specific by age, but there’s no age at which you can’t get LTSS in Colorado.

2:38 And then finally, you have to be on Colorado’s Medicaid program to get the Long Term Services and Supports. And you can get on Medicaid through PEAK. And there’ll be a link to PEAK, your County Department of Human Services, or Medicaid Application Assistance site.

2:53 So now that we’ve discussed who may qualify, I’ll give a brief overview of some of the many services a person may qualify to receive with LTSS.

3:03 As I said at the start, LTSS provides valuable services to enable people to get out and about in our neighborhoods and communities.

3:10 Perhaps you have doctor’s appointments to help with your medical needs, but what about life outside of your health care?

3:16 For example, everyone needs to take care of grocery shopping. Perhaps you’re not sure how you’ll get to the grocery store.

3:22 Services can also be personal care to help with dressing, or to help you get around town.

3:27 Lots of people with disabilities need help keeping our homes clean, due to either physical or mental disabilities, and all of the programs that we call waivers include homemaking as a benefit.

3:38 Other people may struggle to make certain judgments or make decisions, and might need help with that. Some people are compelled to harm themselves, or others have difficulty with self-control and managing aggressive behavior, which then leads to troubles with neighbors or landlords.

3:54 Once again, LTSS can provide support to address issues like these and many, many more, all while keeping Coloradans with disabilities out of any kind of institution.

4:04 Having heard about some of the many important services that are offered, you’re probably wondering how you can get access to Long Term Services and Supports.

4:13 As I mentioned earlier, you have to receive Colorado’s Medicaid as your health insurance provider. But if you’re not already on Medicaid, start by applying to Medicaid. We’ve included links to the Medicaid application in both English and Spanish descriptions below.

4:28 If you’re not sure of whether Medicaid’s your insurance provider, we’ve included a link below that will help you find out if you’re already on Medicaid.

4:36 Again, if you have Medicare or other insurance, you can have Medicaid too. So don’t let that stop you from applying.

4:42 If you’re already on Medicaid or have applied for Medicaid, the next step is to contact your local case management agency to ask about Long Term Services and Supports. We’ve included a link in this description of the video to help you find your local case management agency and how to contact them.

5:00 Your case management agency will then work with you to start an assessment process to determine whether you qualify for LTSS. The assessment process involves assessing or determining, in other words, what kind of daily functional activities a person needs assistance with from a care provider, as well as how often they might need assistance, and how much assistance they need to complete certain activities like the ones we mentioned here, as well as many other everyday activities that we didn’t talk about in this video.

5:29 A caseworker will ask you questions that are designed to help gather information about your needs.

5:34 You can answer these questions on your own, or with the assistance of a guardian, parent, family member, friend, or with the help of an authorized representative of your choosing.

5:45 To be clear, you have control over who gives information and input about your needs during the assessment process.

5:51 When you undergo the assessment process, please be as open and honest and thorough as possible about the challenges you experience and the needs you have.

6:00 The answers you provide to these questions will inform what Medicaid calls the Level of Care that you need, and the specific level of care determines whether you’re eligible or not for Long Term Services and Supports.

6:13 After your assessment, your case management agency will look at the specific level of care you need to find out which waivers you may qualify for.

6:21 In context, the term waiver simply means the type of additional services that you might qualify for on top of your regular Medicaid benefits. Each waiver program is targeted towards a specific disability type, and there are some waivers for children and others for adults.

6:37 We showed a lot of information here about what Long Term Services are and who may be eligible for Long Term Services, and the process to qualify for these services.

6:48 We know this might be overwhelming or confusing, but remember, there are resources to help you with this process.

0:25 the Consumer Directed Attendant Support Services, or CDASS, as most people refer to it.

0:33 When we use CDASS, we act as the decision- makers on how we receive our services, which differs from working with an agency to receive our services.

0:46 With CDASS we are the people who choose and hire the attendants that work

0:52 best for our needs related to personal care, homemaking, and health maintenance services.

1:01 After we hire our caregivers, who are referred to as attendants, we then get to train and manage those attendants,

1:11 so that we can continue to have control and choices for our services.

1:16 Importantly, CDASS also gives us our own individualized budget based on our needs,

1:23 and we are able to use our Medicaid dollars to pay the attendants of our choice, rather than the home health agency or personal care agency.

1:35 Of course, there are certain laws regarding how Medicaid dollars are spent, but CDASS provides us with all of the information we need to make those decisions about our own budget.

1:48 You are the client, and the employer, and you get support and training on how to do that.

1:57 Because CDASS features make us the decision-maker regarding our services,

3:51 The case manager will refer you to training and give you the necessary paperwork to enroll.

4:04 You will learn about Financial Management Services, FMS, and be able to choose an FMS provider.

4:12 An FMS will assist you with payroll and paperwork for your providers. All training is available in person or remote.

If you have a disability that just affects your ability to complete common and daily activities, you may qualify for services you can receive in the community, and the comfort of your own home or wherever you are living.

Colorado Medicaid disabilities programs can provide eligible individuals who have disabilities with what they are known as long term service and supports or LTSS.

In this video, we will give you an overview of what is called the level of care a person must require in order to receive LTSS.

Just so you are aware, level of care is also sometimes called the nursing facility level of care or hospital level of care. But don’t worry, you don’t have to have a hospital or nursing home stay to meet the level of care or access these services.

It can be more costly to receive services at a nursing facility or a hospital, and data shows most people prefer to be in their homes.

Essentially, when you hear people refer to level of care, that simply means you need a certain level of assistance to qualify for LTSS, hence the purpose of this video.

So what is the first step you need to take in order to receive LTSS?

Well, first you must receive Colorado Medicaid.

Coloradans with disabilities can get Medicaid by either applying for waivers through SSI, or through the Working Adults With Disabilities, WAWD.

You must get your health insurance through one of these programs to be eligible for LTSS.

Having explained how to receive LTSS let’s look more closely at how LTSS can support you. There are several common daily activities that LTSS recipients can get help with.

Medicaid refers to these tasks as activities of daily living or functional activities.

Again, to meet the level of care required to receive LTSS you need to require assistance, whether physical, verbal or technological, on a regular basis with some of the activities I am about to discuss.

When I stay regular basis, that means you generally need help more days than not.

Now let’s discuss the various activities of daily life a person might need assistance with.

Physical activities of daily living include your general mobility, meaning your ability to physically get around with or without an assistive device.

Some people find bathing challenging. Some people need help with bathing because they need to be reminded to bathe, or need help getting completely clean.

Another daily living activity involves dressing yourself. Some individuals require assistance completing daily toileting activities. This also includes bowel programs to help with catheters or ostomies.

Eating is, of course, a very important activity of daily life, and some people may need help feeding themselves, or managing their feeding tube.

Other people could benefit from having help with their behavior and emotions.

People may also need help with cognition if they have challenges with their money or clear thinking, while others have disabilities that affect their executive functioning, and could use help with planning, scheduling or problem solving.

Still, other people need help due to sensory disabilities, like being blind or having communication needs, based on your disabilities.

All of the daily activities I just mentioned are absolutely necessary for any person to accomplish in order to live a healthy life, and LTSS can make these tasks easier if you qualify for them.

So how do you know if you qualify to receive LTSS?

When you qualify for Medicaid, you can call your local case management agency and they will assign you a case manager. Your case manager can then give you an assessment to determine whether you qualify for specific waivers under your Medicaid program.

To be clear, a waiver is simply an extra set of non-medical benefits that people with disabilities may qualify for.

Waivers are targeted towards different types of disabilities, so the services differ in some waivers. Some people are eligible for more than one waiver and should be told about the different options and given a choice, because you can only be on one waiver at a time.

When your case manager gives you a level of care assessment for a waiver, it is important to know what to expect so that you get the care you need. An assessment is simply a list of questions to help you identify your needs.

The answers you provide to the assessment questions determine not only whether you are eligible to receive LTSS, but also the amount of care for which you qualify.

Please make sure you are completely honest and share not only what a good day looks like, but how much support you need on a bad day. Many of us do not like the idea of wanting or needing help. The assessment questions can make us feel vulnerable or embarrassed, and some of the questions are about really personal tasks, like bathing and toileting.

I have two pieces of advice to help manage feelings you might have about the assessment. First, if you are concerned about feeling embarrassed or vulnerable, the best way to get past these feelings is actually to get the help you need, which is exactly what can happen if you get LTSS. So often we think that by rejecting help, we are somehow more independent. But every person, no matter their disability status, needs support in various ways.

The second piece of advice I would like to offer you is to have someone you trust with you during the assessment. Someone who will be there to support you, and encourage you to answer assessment questions as honestly and thoroughly as possible, especially if you are concerned about being vulnerable or embarrassed. This person should be someone who knows you and who you feel comfortable around, like a friend, a family member, or an advocate.

We also recommend that you consider giving them permission to remind you if you forget to share something about your needs. Going through the level of care assessment may be challenging, but this is how you open the door to many services that make your life easier.

If you qualify, meet the level of care, the next step in the LTSS process is working with your case manager to create your service plan. Once you have completed your level of care assessment, please know that better things are on the other side of that process for you.
