If you or a family member is on MEDICAID, we want to know what you think about your health care AND about Medicaid.
We want to know what works and what can be better.
If something is NOT working for you, we want to help solve the problem.

PLEASE RSVP TO LET US KNOW IF YOU CAN ATTEND. When you RSVP be sure to let us know if you need an accommodation including a sign language interpreter, alternative format material or anything else. The location will be wheelchair accessible. A light supper and a small gift will be provided to those in attendance.

If you live on the Western Slope and you are on Medicaid and have been enrolled into the Accountable Care Collaborative (ACC), your health care is coordinated by Rocky Mountain Health Plans. Rocky Mountain Health Plans believes that clients should have a say in how health care is delivered. They want to hear from you and have contracted with the Colorado Cross-Disability Coalition to facilitate this event.
